Printing Instructions

To print this document, set all margins to 0.5 inches, use a medium or small size font, and change your printer setup to "landscape." You may fill in the form while it is on your screen. However, if you run into problems (not enough room), it will probably work better if you print a blank form and fill it in manually. The conventional form is available at the EHS office; call 744-7241 and we will send you blank forms.

If you wish, you may devise your own form as long as it contains the same information as this one. You can even e-mail it to us as long as the form was saved as an Excel 5.0 spreadsheet.  Or you can download this blank CIL form (Excel 5.0).
