OSU Construction Practices
During the planning stage of any project, an Asbestos Abatement request form shall be submitted to EHS for the purpose of surveying the proposed project site for the presence of asbestos-containing material.
Request forms are available on-line at the EHS office.
If the Work Control Center submits a project to EHS for estimate, EHS will submit the Asbestos Abatement request form as part of the estimate report. The completed Abatement Form shall be attached to the final Plans Review. Without this document, a Construction Permit shall not be issued.
Projects submitted by OSU A&E Services: The Asbestos Abatement Request form shall be submitted by the responsible Architect.
